1. Release of Student's Records
Collin will release information from a student's
educational record only with prior written consent of the student. See
FERPA. Exceptions to right to consent are published in the college
catalog.
2. Verification of Enrollment
Students requesting enrollment verifications for insurance or other purposes should fill out the Request for Verification of Enrollment form at any of our Admissions and Records offices.
Verifications of enrollment cannot be certified until after the census date for each term. Student classification is based on the student's status at the time of request. Each change in status will be reflected on the form with beginning date, ending date, number of hours and status.
3. National Student Clearinghouse
Student Self-Service Enrollment Verification (online)
This program allows students the ability and ease to instantly obtain an enrollment verification from the National Student Clearinghouse.
Students can achieve the following:
Print a
certificate of enrollment that can be forwarded to a health insurer, housing
provider, credit issuer, employment agency, or other student services providers.
View enrollment
information that may have been provided to a student service provider.
View electronic
notifications and deferment forms that have been sent to lenders, service
providers, and guarantors.
View a list of
their lenders and link to real-time student loan information detail, such as
outstanding principal balance and the next payment due date that some lenders
provide.
Students requesting enrollment verifications for student loan deferments, please visit Collin's Webline (beginning in Fall 2008: CougarWeb) and login under Student Services to request an online verification. Password is required.
4. Change of Name/Change of Address/Change of Social Security Number
Changes of name, address, social security numbers, etc., must be reported promptly to the Admissions and Records Office. A copy of your social security card is required to change your name or social security number. This enables students to receive registration and other information from college departments and programs.
Changes of address affecting residency classification should be promptly reported, along with documentation of current address.
Name and social security number changes require photocopies of the student's valid driver's license and/or social security card.

5. Request for Degree Plan/Certificate/Change of Major
Students can request a degree plan/certificate/change of major by filling out a form in any of our Admissions and Records Offices.A Degree Plan can be completed when:
student is currently admitted to Collin
all official transcripts from each college attended are on file
student specifies the catalog year for the degree requested
Request for Degree Plan form (pdf version - 2 pages). Please print, complete form and fax to 972.377.1723 (Preston Ridge Campus).

6. Petition for Degree/Certificate
Candidates for an Associates degree or Certificate should submit an Petition for Graduation by the deadline established for that semester.Students may fill out a Petition for Degree/Certificate form in any of our Admissions and Records offices. Please print, complete form and fax to 972.377.1723 (Preston Ridge Campus). For additional information, see Graduation.
7. Class Schedule Printout
Students needing a printout of their schedule of classes should bring a photo ID to the Admissions and Records Office. Written authorization by the student must be provided to release a class schedule to another person.
8. Personal Identification Number (PIN) for Summer 2008 only
Students who have completed the admissions and advising process can get their PIN for online (Webline) registration at the Admissions and Records Office (ARO).The PIN issued will give students access to the online system for registration. Effective August 5, 2004, PINs issued to students are valid for one time only. Students are required to change the number the first time it is used to a 6-digit number (no letters). Once the number is changed, the ARO will no longer be able to release a PIN if the student forgets or loses the PIN.
Students who lose or forget the PIN they choose will be required to visit any of our campuses and provide a photo ID (Texas driver's license or student ID) and get a new assigned PIN that will still be valid for one time only.
Students can also request their PINs by
email. Requests
should be sent to:
cougarweb@ccccd.edu. Requests should include full legal name,
date of birth, and an email address that must resemble the student's name.
The Admissions and Records Office (ARO) is responsible for protecting student's
records. Therefore, ARO will not issue PIN numbers by telephone or fax.
9. CougarWeb Password (beginning Fall 2008)
CougarWeb is the new portal for students
beginning with Fall 2008 registration.
Current students will sign on using their current CourgarWeb ID (which is
the first part of their Cougar email address i.e. jsmith138). The password is
the last six digits of the Campus Wide ID (CWID).
New students to Collin will automatically
get an email from ARO that provides the following information:
CougarWeb username
CougarWeb password
CougarMail address
CWID
When logged into CougarWeb for the first time, the student will be asked to
create a new password along with challenge questions used to help retrieve a new
password should it be forgotten at a later date.
10. Senior Citizen's Waiver
Students
age 55 and over (as of first class day) who are Texas residents are eligible for
lower tuition rates. Proof of date of birth is required.
