NURSING PROGRAM TRANSFER APPLICATION CHECKLIST

Submit the following for general admission to CCCCD:

______ Completed general admissions application

______ Official college/university transcripts from all schools attended
If no college, then official high school transcript or GED required

Course descriptions required for out-of-state/private college.

Proficiency Tests:

______ PSB Nursing School Aptitude Exam (see schedule)

Submit the following to the Nursing Office:

______ Completed Nursing Program application
______ Official college/university transcript(s) Copies will be accepted from CCCCD’s admissions office, however, applicants must request that transcripts be forwarded to Health Sciences Division, Nursing Program. This includes a CCCCD transcript.

______ Copies of course syllabi and college catalog from previous nursing program
______ Two letter of recommendation from previous program’s faculty and/or director

Pre-requisite courses which must be completed:*

______ Statistics**
______ Anatomy & Physiology I**
______ Anatomy & Physiology II**
______ Microbiology**

*Check with college admissions for acceptance of courses from other colleges.
**These courses must be completed within 5 years of application deadline and with a 2.5 GPA.
